I am agreeing to abide by the Rules of the Kildare & District Underage League Soccer academy. I am authorising the Academy personnel to bring my child to hospital in the case of emergency (having been unable to contact either parent on number supplied) and to take all medical/surgical procedures as advised by a treating doctor. I agree that The Academy may video/photography my child. Any video or photography will be used for the benefit of the Academy and may be used for promoting the Academy. I am authorising the Academy staff to make direct contact with my child in respect of Academy activities. All relevant information may be retained by the Kildare and District Underage League for information purposes only.
I agree to be bound by the Code of Conduct as set out in the rules of the Kildare and District Underage League Academy.
