I am aware that Gaeil Colmcille has implemented a Child Protection Policy, which is designed to protect Youth Members and all others who join the Club.

I have seen the induction booklet and am aware of the Code of Ethics / Discipline Code that is expected by this Club.

I commit to assisting the Club in any way I can with regard to the promotion and development of the Games and the implementation of the Code of Best Practice.

I will make the club aware of any relevant MEDICAL HISTORY INFORMATION (details of any known allergies, conditions, medications). Failure to do so is my responsibility

In the event of illness, having parental responsibility, I give permission for medical treatment to be administered where considered necessary by a nominated First Aid person, or by suitably qualified medical practitioners. If I cannot be contacted and my child needs emergency hospital treatment, I authorize a qualified medical practitioner to provide emergency treatment or medication.

Consent for activities & away trips:

I undertake to pay any reasonable sums for transportation to or from games or activities. I also undertake to pay the required sums by the dates specified in information supplied and accept that in respect of any withdrawal from the trip, for whatever reason, there can be no refund of the whole or part payments unless the circumstances are covered by insurance. I undertake that where possible I will assist the club with transportation of juvenile members with the permission of parents / guardians and coaches / mentors. I undertake to ensure that all juvenile members are adequately restrained via seat belts and that I hold necessary driving license and insurance.

I accept to receive details of any activity and consent to my child taking part in the visits and activities indicated. I acknowledge that the Club will be liable in the event of any accident only if they have failed to take reasonable steps in their duty of care for my child during the trip. I understand that the Club has a common law duty to act in the capacity of a reasonably prudent parent/guardian.

Photographs

I understand that photographs will be taken during or at sport related events and may be used in the promotion of the club, and published online, and I consent to the taking of such photographs.

I have read the Code of Best Practice and agree that my child should abide by this whilst in the care of the Club and I understand that a serious or continued breach of this Code may result in my child being disciplined by the club, and may result in the child being asked to leave the club. This includes any incidents of bullying, (including online)

I hereby consent to the above child(ren) participating in activities of the club in line with the Code of Ethics for Young People. I will inform the leaders of my children’s activities of any changes to the information above. I confirm that all details are correct and I am able to give parental consent for my child(ren) to participate in and travel to all activities.
